WorkSafe New Zealand considers that there are health and safety risks arising from SCUBA and SCBA cylinders manufactured from aluminium alloy 6351, and has withdrawn approval for cylinder designs which used this alloy. Testing and filling stations should not test or fill any SCUBA or SCBA cylinder made from aluminium alloy 6351 listed in the table overleaf from 31 October 2017. A voluntary product check of select regulators with the ACD yoke systems is being offered by Aqua Lung. This concerns three regulators that may have a component in the ACD yoke system that may not have been tightened to the recommended torque when manufactured. This may lead to a possible gas flow failure during a dive when the regulator has been fitted on certain valves. Many countries have a rule requiring that high pressure cylinders undergo a hydrostatic requalification on a scheduled basis.  This article refers to the Hydrostatic requalification of cylinders, but also mentions that during this requalification, a visual inspection must be performed. The Hydrostatic requalification periods may vary amongst high pressure cylinders, but a common time frame is every five years. This infers that a cylinder gets some visual inspection every five years, even though cylinders may be exposed to safety concerns countless times within a requalification period. I recently received a call/e-mail from an instructor about an unsafe situation. She stated that one of her employees recently took a trip and had their SCUBA cylinders filled at a remote location. The other facility filled their cylinder to 5000 psi (345 BAR) when they were rated to 3000 psi (206 BAR). Her staff was obviously concerned and drained the cylinder to 2500 psi (172 BAR) the moment they recognized the mistake. However, that brought up the question of what to do next.
